MySQL host, user, password and database name

The installation script (step 1) cannot create a MySQL database and a MySQL user for you. You have to do this from your hosting panel. If Cpanel - go to Cpanel ยป MySQL databases:

MySQL host is in most cases localhost - if you are unable to connect to your database you'll have to ask your hosting provider about this. Cpanel usually adds a prefix to the MySQL databases and users which is visible but cannot be changed. When you submit a MySQL database and a MySQL user during the installation process you have to enter both names with the prefix - xxxx_dbname & xxxx_user where xxxx is most likely your Cpanel username. No prefix for the password.

CHMOD

CHMOD means to change the permissions (Read, Write, Execute) of a file or a folder on a Linux computer. You can do this easily with an FTP program - right click on a file or a folder and look for File permissions. Most FTP programs provide a field to enter a numeric value.
